All new students entering a CCSD 46 school must have an Illinois school physical form and vision exam on file.
Sports physicals are required for all students who plan on playing sports at Grayslake Middle School and Park Campus. Forms are available in the school offices, and must be on file at school in order to tryout for a team.
All students entering Pre-K, Kindergarten or 6th grade must have a school physical form on file.

SPORTS MEDICINE/CPR TRAINING
State law requires the Illinois High School Association to post a hands-only cardiopulmonary resuscitation and automated external defibrillators training video on its website. The law also requires the District to notify staff members and parents/guardians about this video. You are encouraged to view the video, which will take less than 15 minutes of your time.
Navigate to the Resources tab, select Sports Medicine, and scroll to the CPR Training link.

District Immunization Summary 20-21
For 2020-2021, the State of Illinois has provided a temporary reporting category "non-compliant but remote due to COVID-19 concerns." This category allows schools to count non-compliant remote learning students without a penalty until the district returns to onsite learning. This temporary allowance may cause these compliance rates to be slightly higher than they are in actuality. Per District policy, all students must be compliant upon return to onsite learning.
